• IUA Process: After the IUA is met, the need remains open until either costs related to that need cease or 12 months pass since the last addition of a medical cost.
• Members enjoy limited financial exposure: In any rolling 12-month period, if three Sharing Requests have reached the Initial Unshared Amount (IUA) level, any fourth eligible sharing request  is shared from the first dollar once it has reached a $500 threshold.

• Sharing Limitations to protect the membership: A pre-membership condition is anything you have had symptoms of, received treatment for, or been diagnosed with in the past 24 -36 months (depending upon medical cost sharing community).

• Graduated sharing on Pre-Membership Conditions as follows:
• Months 0-12 - No sharing
• Months 13-24 - Sharing up to 25K
• Months 25-35 - Sharing up to 50K
• Month 36 and beyond - Sharing up to $125K annually

• Exception for High Blood Pressure, High Cholesterol and Diabetes type 1 and 2: These conditions are accepted as long as there hasn't been a hospitalization for the condition in the past 12 months and it can be controlled through medication or diet.
• Arthritis: Considered a pre-membership condition and gradually shareable according to member guidelines. This includes treatment and procedure costs including joint injections, PT, and chiropractic treatments. 
• Cataracts: Eligible after first year of membership. View sharing guidelines

• Maternity: Maternity requires a 6 month waiting period –Maternity and birth is one IUA

• Smokers:  There is a monthly surcharge per household for tobacco users and vapers, view sharing guidelines.
• Smokers over age 50 there is a sharing limit of $50,000 for heart conditions, cancer, COPD and stroke.
• Substance Abuse: treatment shareable per guidelines

• Perimenopause and menopause: Yes, Providers can evaluate symptoms and discuss treatment options and management.
• Menopausal hormone therapy/HRT: Yes, when clinically appropriate. This can include evaluation and management of menopausal symptoms.
• Estrogen and progesterone: These may be prescribed when medically appropriate and within the provider’s clinical and prescribing guidelines.
• Men’s hormonal health / low testosterone: Providers can evaluate symptoms and order appropriate labs. Testosterone is a DEA-controlled medication, so it is not prescribed through our virtual primary care service.

• Thyroid and other routine hormonal conditions: Yes. Providers can evaluate symptoms, order appropriate laboratory testing, and manage routine conditions when they are within the scope of virtual care.
• Dermatology: Common skin concerns can be evaluated virtually, and appropriate prescriptions may be provided when clinically indicated. Lyric also offers dedicated virtual dermatology for conditions such as acne, eczema, and other common dermatological concerns.
• Laboratory testing and follow-up care: Yes. Providers can order labs when medically necessary, and our integrated care model supports laboratory services and ongoing follow-up.
