2024 Preventive Reimbursement Benefit Maximums and Eligible Services

Colorectal Cancer Screening:
- Age range: 45 - 80 (covered from age 40 if 1st degree relative has positive history of colon cancer)
- Screening Type: Procto/Sigmoidoscopy every 5 years. Cologuard every 3 years.
- Colonoscopy is an eligible screening type
Bloodwork Screenings:
- Lipid Panel (Cholesterol and triglycerides)
- Comprehensive Metabolic Panel (CMP)
- Complete Blood Count (CBC) with differential test
- Thyroid Panel TSH (Thyroid Stimulating Hormone) with reflex to T4 if abnormal
- Routine Comprehensive Urinalysis (UA)
- Hemoglobin A1C (HgbA1C)
- Vitamin D Level
PSA for men age 50-75 Start at age 40 if 1st degree relative has positive history of prostate cancer
Bone Density Screening Post-menopausal women, every 2-3 years
Heart Screening: C-Reactive Protein (CRP) & Homo cysteine levels for those with strong personal history or 1st degree relative history of
heart disease or those of South Asian or Pacific Island origins.

Preventative Services Included with MPB.Health DIRECT
Commencing Day 1 of membership
- One outside visit worth up to $175 - no waiting period
- Pap smears will be considered fully shareable when billed alongside the annual provider visit.
- Alternately, use your outside visit for any service or care, including vision or dental, mental health visit, etc.
Commencing after 6 months of membership
- Annual Screening Mammograms - 6 month waiting period - cap at $600 and no network concerns - Age 40 - 2D or 3D. The exam, appointment and evaluation as part of the screening counts towards the $600 limit. High risk members under the age of 40 may be considered with prior approval. If findings warrant an ultra sound, this may be considered for sharing with prior written approval.
- Screening Colonoscopy - 6 month waiting period - cap at $5,000 - no network concerns - Age 45 - available every 10 years. 5K includes screening, anesthesia, diagnostic texting, biopsies and pathology performed as a part of the screening.
- For high risk members under age 45, colon screening may be eligible for screening with prior approval
- Home colorectal cancer screening tests such as high sensitivity gFOBT (guaiac fecal occult blood test), FIT (fecal immunochemical test), and sDNA -FIT (stool DNA with fecal immunochemical test) are shareable beginning at the age of 45 and after six (6) months of continuous membership with preventive services.
- The gFOBT and FIT are shareable once every twelve (12) months; sDNA-FIT is shareable once every three (3) years up to $500 for the test.
- Childhood Well visits & Immunizations to age 18 - 6 month waiting period (unless pregnancy was also shared - then no waiting)
- Well baby Visits at: Birth, Month 1 and Months 2, 4, 6, 9, 12, 15,18,30 and 36.

- Use the PHCS MultiPlan Network - this is by far the easiest and best path for the community as these rates have been per-negotiated.
- If you do not see your provider in Network, MPB Health has a back up solution. They also allow for out of network. Simply tell your provider to call the number on the back of the card.
