- Enter oldest members information as the primary
- Enter spouse and or children if applicable
- Hit update.
- Choose Initial Unshared Amount from the product selection with the drop down button
- Hit continue
- You will now be on the second page. Enter primary members demographics, address, etc.
- Please note that the primary persons email will be the administrative email write
- Please indicate the person that referred you
- If you have a spouse or children that will be on this membership click on the pencil icon that is to the right hand side of the screen.
- Start with a spouse. Clicking the pencil will auto fill most of the required information.
- You will need to enter Gender, Social and a separate email - this email will be their access email to the included unlimited virtual care
- You do not need to complete more than this if the spouse has the same mailing address.
- Hit SAVE Member. This then gets entered.
- Next do the same for each child as applicable, then hit save member.
- The information will get entered in as you go along - it is there even though you dont see it after hitting Save to allow for multiple inputs
Next:
- Complete the Principles of Membership. You must agree yes with all questions
Next comes the Medical History:
- The answer is yes that you understand that there is a pre-existing conditions clause for anything that you have had diagnosis of, treatment for or symptoms of in the last 24 months.
- Please note that Diabetes 1 & 2 as well as High Blood Pressure and High Cholesterol is NOT considered a pre-existing condition if it is controlled by either diet or medication and there has not been a hospitalization because of this issue during the last 2 years.
- You will also respond yes to the next question regarding pregnancy. You acknowledge that pregnancy related expenses will not be shared if conception is pre-existing or occurs within the first 2 months of membership. (so be careful!)
- Next you will disclose any pre-existing conditions you have had in the last 2 years. If nothing - write N/A
- Same for spouse, children
Authorize, Click Humanity, Enter your credit card (3 percent surplus) or Direct Withdrawal (charge will occur on 20th of month for monthly share and on enrollment for Initial set up cost) I highly suggest the direct withdrawal. Why pay extra?
Attempt a signature of the primary person on the applications name. Anything works - it is hard to do on a computer. Type the primary member's name and hit submit.
If successful you will receive a success page.
The next step is to wait for your Welcome Email from PBarron@mympb.com
You need to choose a time to meet with the Concierge as early as possible after your membership begins. You can choose the time now - but it must take place after you are an active member.
You will see the hyperlink to schedule that call in blue in the center of the email.
If you are joining SECURE you will receive an additional email requesting the required documentation. Follow the prompts. Ignore this email if you are part of an employee group.
Concierge sets aside a 1/2 hour welcome call with you to review all aspects of your membership.
